英文原摘要(English Abstract)
Effective disease-modifying therapies for knee osteoarthritis (KOA) remain elusive. This study developed BWTS-N@Gel, a novel composite comprising supramolecular self-assembled nanoparticles (BWTS-N) derived from the traditional Chinese medicine formula Bawei Tiaosui Decoction, encapsulated within a cross-linked hyaluronic acid hydrogel. BWTS-N features a small particle size facilitating efficient chondrocyte endocytosis, while the hydrogel matrix ensures sustained drug release, and the incorporation of BWTS-N further enhances lubrication, aligning with the "SCALE" therapeutic strategy (Safety, Cartilage protection, Anti-inflammatory, Lubrication, Efficacy). In vivo and in vitro experiments, along with ex vivo human cartilage explant studies, demonstrated that BWTS-N@Gel significantly attenuated KOA progression by suppressing chondrocyte ferroptosis. Mechanistically, the treatment preserved the GCLM/GPX4 axis, thereby enhancing glutathione synthesis, mitigating lipid peroxidation, and preventing iron-mediated cell death. These findings position BWTS-N@Gel as a promising therapeutic candidate that synergizes traditional medicine efficacy with advanced nanotechnology for targeted cartilage protection and joint lubrication in KOA management.

This study aims to comprehensively elucidate the therapeutic mechanisms of Huanglian Jiedu Tang in bacterial meningitis via an integrated strategy combining network pharmacology, molecular docking, and experimental validation. Network pharmacology was utilized to screen for bioactive constituents, pivotal targets, and associated signaling pathways of HLJDT. The potent candidate Obacunone and its target MAPK14 were prioritized for verification. An in vitro bacterial meningitis cell model was established, followed by therapeutic interventions using Obacunone, the MAPK14 inhibitor VX-702, or their combination. Therapeutic effects were assessed using the CCK-8 assay, sodium ion permeability assays, TUNEL staining, RT-PCR, Western blot, and ELISA. Bioinformatics analysis of the GEO dataset GSE40586 identified 495 BM-related genes, 30 of which intersected with HLJDT targets. GO and KEGG enrichment analyses highlighted biological processes including antibiotic and glucocorticoid responses, as well as transcriptional misregulation and HIF-1 signaling pathways. Molecular docking simulations revealed that Obacunone possessed superior binding affinity to MAPK14. In the BM cell model, treatment with Obacunone or VX-702 alone significantly improved cell viability, attenuated the increased sodium ion permeability, inhibited apoptosis, and suppressed inflammatory cytokine levels. Notably, combination therapy exhibited the most potent synergistic effects. In a bacterial meningitis co-culture model, combined treatment with Obacunone and VX-702, as well as MAPK14 silencing, significantly restored the viability and migratory capacity of HMC3 cells while effectively attenuating early apoptosis. 英文原摘要(English Abstract)
[INTRODUCTION] Huangqin decoction (HQD), a traditional Chinese medicine prescription, is used to treat gastrointestinal diseases, including ulcerative colitis (UC). However, systematic research on the components of HQD remains insufficient. Therefore, we aimed to perform chemical profiling, network pharmacology, and bioinformatics analyses of HQD to identify candidate constituents potentially associated with UC and to establish a quantitative method for their determination in HQD. [METHODS] Qualitative chemical profiling was performed to identify 51 compounds in HQD, and their potential targets were predicted. UC-related target genes were identified by combining results from public and Gene Expression Omnibus (GEO) databases. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were performed to investigate the biological processes and signaling pathways associated with UC. Moreover, protein-protein interaction (PPI) analysis was performed. Based on these results, 15 putative active constituents of HQD were selected and quantified. Molecular docking analysis was then performed to evaluate the binding interactions between these compounds and key target proteins. [RESULTS] A total of 947 HQD component-related, 1,868 UC-related, and 2,930 GEO database-related target genes were intersected to obtain 109 common target genes for HQD and UC. GO and KEGG enrichment analyses indicated that these targets were mainly associated with inflammatory and immune-related biological processes and signaling pathways. Among the identified targets, NOS2, AHR, MMP3, MMP9, and PRKCQ were highlighted as potential key targets. The analysis of three batches of HQD samples revealed that baicalin had the highest content. Molecular docking results indicated favor
able predicted interactions between the putative active compounds and core target proteins, with several compound-target pairs exhibiting docking scores below -11.0 kcal/mol. [DISCUSSION] This study not only provides a comprehensive chemical profile of HQD but also presents a new approach for evaluating and managing quality based on putative active constituents. These findings may serve as a scientific basis for further pharmacological and clinical studies.

英文原摘要(English Abstract)
[OBJECTIVE] To investigate the specific mechanism by which Ruanjian Sanjie Formula (RJSJ) inhibits the occurrence and metastasis of lung cancer. [METHODS] This study developed an in vivo model using H460 cells in nude mice to investigate postoperative lung cancer metastasis. Two groups received either RJSJ (0.2 mL) or saline to assess RJSJ's anti-cancer effects, including disease-free survival duration, recurrence-free metastasis rate, number of pulmonary metastases, as well as weight and coefficient of major organs. An in vitro study was conducted to evaluate the effects of RJSJ at concentrations ranging from 0 to 500 µg/mL on the proliferation, migration, and invasion capabilities of H460 and H1975 cell lines. These effects were assessed using the CCK-8 assay for cell proliferation, the wound healing assay for cell migration, and the Transwell assay for cell invasion. RJSJ's chemical profile was analyzed via UPLC-Q-TOF-MS, and its mechanism was explored through network pharmacology. Specific microRNAs and their target genes were identified and analyzed using bioinformatics, qRT-PCR, and Western blot to examine RJSJ's impact on metastasis-related genes and proteins during epithelial-mesenchymal transition (EMT). Virtual docking and molecular simulations were used to evaluate interactions between RJSJ compounds and microRNAs. [RESULTS] RJSJ could inhibit the proliferation, migration, and invasion of lung cancer cells in vitro (P<0.05 or P<0.01), and it had also been shown to prevent lung cancer recurrence and metastasis in vivo (P<0.05); it contains 47 chemical compounds that act on 191 common targets, including 36 core targets. It reduced cancer cell proliferation and invasion, downregulated miR-182-5p, thereby regulating its target gene EPAS1, and regulated EMT-relate
d markers by decreasing the expression of N-cadherin, MMP2, MMP9, Snail, and Twist, while increasing the expression of E-cadherin. Chlorogenic acid was identified as the main anti-tumor component due to its strong binding affinity to nucleic acids. [CONCLUSIONS] RJSJ inhibits lung cancer occurrence and metastasis both in vivo and in vitro. Mechanistically, RJSJ suppresses the EMT process by downregulating miR-182-5p and inhibiting EPAS1 expression, thereby restraining lung cancer malignant progression, with chlorogenic acid identified as a key active component.

[OBJECTIVE] To determine whether radiation directly compromises AT2 cell stemness through oxidative and DNA damage, and to evaluate whether Compound Kushen Injection (CKI) protects AT2 cells, restores alveolar regeneration, and mitigates radiation-induced pulmonary fibrosis. [METHODS] Mice were randomly assigned to a control, irradiated model, or CKI-treated group. CKI (4 mL·kg-1·d-1) was administered intraperitoneally from 3 days before irradiation until the designated endpoints. A thoracic irradiation plus left pneumonectomy model was established to mimic clinical scenarios requiring postoperative lung regeneration. Lung function, histopathology, and compensatory growth of the accessory lobe were assessed. AT2 cells proliferation, differentiation, and transitional Krt8+ accumulation were evaluated using lineage tracing in Sftpc-CreER and Rosa26-ZsGreen mice. Oxidative stress and DNA damage were examined by immunofluorescence. Transcriptomic profiling of sorted lineage-labeled AT2 cells was conducted to identify radiation- and CKI-responsive genes. [RESULTS] Radiation caused marked oxidative stress and γ H2AX-defined DNA damage in AT2 cells, accompanied by suppression of Sod1 and Parp1 and increased Il6 and Tgfb1 expression (P<0.01). These epithelial-intrinsic injuries led to reduced AT2 proliferation, impaired AT2-to-AT1 differentiation, and long-term accumulation of Krt8+ transitional cells. Functionally, irradiation significantly impaired compensatory lung regeneration after pneumonectomy and promoted fibrotic remodeling. CKI markedly alleviated ROS accumulation and DNA damage in AT2 cells, restored Sod1 and Parp1 expression, suppressed inflammatory and profibrotic signaling, rescued AT2 proliferative and differentiative capacities, reduced transitional Krt8+ cell a
ccumulation, and improved both alveolar regeneration and fibrosis (P<0.05). [CONCLUSIONS] CKI preserves epithelial integrity by reducing oxidative and DNA damage, restoring AT2 stemness, and enhancing alveolar repair. These findings provide mechanistic support for CKI as a potential epithelial-directed therapeutic strategy for preventing or treating radiation-induced lung fibrosis.

Yupingfeng powder (YPFP), also known as Yupingfeng San, is a traditional Chinese medicine (TCM) formula comprising Astragali Radix, Saposhnikoviae Radix, and Atractylodis Macrocephalae Rhizoma, and is used clinically to treat renal diseases in China. Chemical metabolites in YPFP play a significant role in specific pathophysiologic mechanisms of renal diseases. Chemical metabolites, clinical application, and pharmacology of YPFP were comprehensively summarized. YPFP and its bioactive metabolites (notably flavonoids, saponins, polysaccharides, organic acids, lactones, coumarins) demonstrate significant pharmacological activities in renal diseases, including anti-fibrosis, antioxidant, immunomodulatory, anti-apoptosis, anti-inflammatory, and anti-ferroptosis. These activities alleviate symptoms and delay disease progression. Over recent decades, extensive research has confirmed the therapeutic benefits of YPFP and its bioactive metabolites in the medical intervention of renal diseases. This review systematically investigates the renoprotective effects of YPFP, including its therapeutic effect and multi-target mechanisms, providing a reference for the therapy of renal diseases.

[OBJECTIVE] To evaluate the clinical efficacy of wrist-ankle acupuncture (WAA) at different time points for pain after haemorrhoidectomy and to identify an optimized analgesic strategy. [METHODS] Patients undergoing haemorrhoidectomy between June 2024 and March 2025 were randomly assigned to three groups: control (n=96, conventional care), immediate analgesia (n=89, WAA at lower zone 6 upon first pain [VAS≥1]), and preemptive analgesia (n=90, WAA at lower zone 6 within 1 hour postoperatively before pain onset). Needle retention was 6 hours in both intervention groups. Primary outcomes were 24-hour pain trajectory, cumulative pain burden (AUC), and peak VAS. Secondary outcomes included postoperative urinary retention (POUR) and additional analgesic requirement. [RESULTS] From 6 to 24 hours postoperatively, the preemptive group consistently showed lower VAS scores than the control and immediate groups. Both intervention groups reduced AUC versus control, with the preemptive group achieving the lowest burden (mean AUC=44.10, 95% CI: 39.08-49.11), representing a 24.5% reduction compared with controls (mean AUC=58.44, 95% CI: 53.67-63.21; MD=-14.34). Peak VAS was lower in the preemptive group versus controls (MD=-0.64, P=0.012), but no significant difference was observed between the immediate and control groups (MD=-0.32, P=0.453). Preemptive analgesia significantly reduced POUR incidence (14.44% vs 29.17% in controls; adjusted OR=0.38, 95% CI: 0.17-0.80). Both intervention groups required fewer additional analgesics than controls (35.42% rescue rate), with the greatest reduction in the preemptive group (13.33%; adjusted OR=0.25, 95% CI: 0.11-0.52), followed by the immediate group (22.47%; adjusted OR=0.46, 95% CI: 0.23-0.90). In patients stratified by types of basic analges
ics (Med1 (Flurbiprofen Esters Injection) and Med2 (Parecoxib sodium for injection) subgroups), the preemptive analgesia group consistently showed significantly lower VAS scores (P < 0.01). During the treatment period, no cases of fainting (needle syncope) or subcutaneous haemorrhage occurred in any group. [CONCLUSION] Preemptive WAA within 1 hour postoperatively reduces 24-hour pain, cumulative pain burden, peak VAS, POUR incidence, and additional analgesic requirement, with a favourable safety profile. It represents a promising non-pharmacological adjunct for multimodal perioperative care; however, multi-centre trials are needed to confirm its generalisability before routine adoption. [BACKGROUND] Depression poses a major global health burden. Many patients exhibit treatment resistance or experience intolerable side effects with standard therapies. Acupuncture shows promise as an adjunctive therapy for depression, but its neurobiological mechanisms remain incompletely understood. [METHODS] We conducted a coordinate-based meta-analysis to identify consistent patterns of acupuncture-induced neuroplastic changes in patients with major depressive disorder, quantified via resting-state functional magnetic resonance imaging measures of regional homogeneity and amplitude of low-frequency fluctuations (ALFF/fractional ALFF). We synthesized data from 7 randomized controlled trials involving 357 patients with major depressive disorder, comparing acupuncture-based interventions (manual acupuncture, electroacupuncture, or transcutaneous auricular vagus nerve stimulation) against sham acupuncture, conventional treatment, or treatment-as-usual. The seed-based d mapping with permutation of subject images algorithm was used for the imaging meta-analysis, and a leave-one-out sensitivity analysis was performed to assess the robustness of the pooled findings to intervention heterogeneity. [RESULTS] Acupuncture induced significant regional homogeneity /ALFF increases within the right cingulum and right caudate nucleus. In contrast, conventional treatments primarily increased activity in the left hippocampus and right middle occipital gyrus. Meta-regression linked clinical improvement specifically to plasticity changes in the left cerebellum and revealed a dose-dependent relationship between the number of acupuncture sessions and modulation of the left amygdala. l conditions. [CONCLUSION] Our findings indicate that acupuncture-based interventions elicit a distinct pattern of neuroplastic remodeling, targeting key regions involved in emotion regulation and reward processing, which contrasts with the hippocampal-occipital effects of conventional treatments. The correlation between neuroplastic changes in affective circuits and clinical improvement supports a potential mechanism for acupuncture's efficacy in alleviating core depressive symptoms such as anhedonia and emotional dysregulation. Limitations include the small number of included trials, heterogeneity in acupuncture protocols and resting-state functional magnetic resonance imaging metrics, absence of long-term follow-up data, and methodological caveats inherent to coordinate-based meta-analysis. Future research requires larger, standardized trials with follow-up assessments and integration of neuroimaging with molecular biomarkers to advance personalized neuromodulation strategies for depression.

[OBJECTIVE] Normal sleep rhythms are crucial for hippocampus-dependent advanced cognitive functions. Chronic sleep deprivation (CSD) impairs hippocampal neurogenesis and structure, leading to neurocognitive deficits. Electro-nape-acupuncture (ENA) at bilateral Fengchi (GB20) and Gongxue (Extra) is a specialized acupuncture technique for treating insomnia, amnesia, and other brain-originated diseases. This study aims to investigate whether ENA improves CSD-induced cognitive impairment by regulating neurogenesis and synaptic plasticity in the hippocampus. [METHODS] The modified multi-platform water environment method was used to establish the CSD model. Electroacupuncture or sham electroacupuncture was used to treat bilateral cervical acupoints (Fengchi and Gongxue) for 20 min, once a day for 14 days. The Morris water maze experiment evaluated spatial learning and memory in rats, and the new object recognition experiment evaluated recognition memory. Immunofluorescence (IF) staining and Western Blot (WB) were used to detect the expression levels of the hippocampal neurogenesis markers, doublecortin (DCX) and Ki-67. Golgi-Cox staining and transmission electron microscopy were used to observe the changes in neurons and synaptic plasticity in the dentate gyrus (DG) of the hippocampus. [RESULTS] Neurocognitive impairment induced by CSD is associated with abnormal changes in hippocampal neurogenesis and synaptic plasticity. The results of IF and WB showed that the protein expressions of DCX and Ki-67 in the hippocampus of CSD rats were significantly decreased. Transmission electron microscopy revealed that in the DG region of the hippocampus of CSD rats, the synaptic density and the thickness of the postsynaptic density membrane decreased, while the synaptic cleft width increa
sed. Golgi staining showed that the density of dendritic spines in the DG area of the hippocampus in CSD rats decreased significantly, especially mushroom-shaped dendritic spines. ENA can enhance spatial learning and memory, as well as recognition memory, induced by CSD, and effectively reverse the abnormal changes in neurogenesis and synaptic plasticity in the DG region of the hippocampus. [CONCLUSION] In male Wistar rats, ENA improves neurocognitive function by promoting neurogenesis in the hippocampal dentate gyrus and restoring synaptic plasticity, thereby reconstructing neural memory circuits. ENA therapy offers a new strategy for treating cognitive impairments related to chronic sleep deprivation in males, and holds potential significance for the clinical management of cognitive impairment diseases.

[OBJECTIVE] To evaluate the short-term clinical efficacy and safety of Linggui Tanxue acupuncture combined with extracorporeal shock wave therapy (ESWT) in patients with piriformis syndrome (PS). [METHODS] Sixty patients with PS aged 35 to 45 years were prospectively allocated using a random-number table into a control group receiving ESWT alone and a combined-treatment group receiving Linggui Tanxue acupuncture followed by ESWT, with 30 patients in each group. The treatment course lasted 2 weeks. Visual analogue scale (VAS) pain scores, musculoskeletal ultrasound (MSUS)-measured piriformis muscle thickness, clinical efficacy category, and adverse events were evaluated. Pain assessment and ultrasound measurement were performed by assessors blinded to treatment allocation. Because the study was not prospectively registered and no predefined statistical analysis plan or formal sample size calculation was established before initiation, the analysis should be interpreted as exploratory. [RESULTS] Baseline sex distribution, age, disease duration, VAS scores, and piriformis muscle thickness were comparable between groups (all p > 0.05). After 2 weeks, both groups showed significant reductions in VAS scores and piriformis muscle thickness (both p < 0.001). The combined-treatment group showed significantly greater improvement than the ESWT-alone control group in the two continuous outcomes: post-treatment VAS score was lower in the combined-treatment group (1.44 ± 0.10 vs. 3.57 ± 0.17; mean difference, -2.12; 95% CI, -2.20 to -2.05; p < 0.001), and post-treatment piriformis thickness was also lower (9.04 ± 0.16 mm vs. 12.16 ± 0.25 mm; mean difference, -3.13 mm; 95% CI, -3.23 to -3.02; p < 0.001). The combined-treatment group also had a significantly higher total effective rate
than the control group (86.7% vs. 46.7%; Fisher exact p = 0.002), but this categorical outcome was considered supportive and should be interpreted cautiously. No serious adverse events occurred. [CONCLUSION] Linggui Tanxue acupuncture combined with ESWT may provide greater short-term pain relief and greater ultrasound-measured improvement in piriformis muscle thickness than ESWT alone in patients with PS. The findings are encouraging but not definitive because of the absence of prospective registration, lack of a predefined statistical analysis plan, lack of formal sample size calculation, and the short 2-week follow-up period. Larger prospectively registered multicenter studies with predefined statistical analysis plans, formal sample size calculations, and longer follow-up are warranted.

[PURPOSE] Acupuncture has shown its efficacy in improving pain-specific disability among patients with degenerative lumbar spinal stenosis (DLSS) who predominantly present with neurogenic claudication pain symptoms. It is still unclear which group of the patients have better response to acupuncture. This secondary analysis of a randomized controlled trial aimed to identify potential prognostic factors and treatment effect modifiers of response to acupuncture in DLSS patients with neurogenic claudication. [PATIENTS AND METHODS] A secondary analysis was conducted based on a multi-center randomized controlled trial, where 196 DLSS patients were randomized to acupuncture or sham acupuncture and 178 completed the 6-week intervention. Responders were defined by a 2-point reduction in the modified Roland-Morris Disability Questionnaire (RMDQ) score from baseline to week 6. Baseline characteristics were summarized using descriptive statistics. Seventeen baseline characteristics were considered as potential prognostic factors of response and subjected to logistic regression analysis. [RESULTS] Among the 178 patients included in this secondary analysis, 119 were defined as responders. Treatment assignment (OR 2.0977; 95% CI 1.0300 to 4.2721; p = 0.0412), age (OR 1.0684, 95% CI 1.0176 to 1.1217; p = 0.0078), and Swiss Spinal Stenosis Questionnaire (SSSQ) symptom severity score (OR 1.1270; 95% CI 1.0077 to 1.2606; p = 0.0363) were positively associated with patient response. Patients with a shorter duration of the current episode (OR 0.9384; 95% CI 0.9020 to 0.9764; p = 0.0017) might specifically benefit more from acupuncture. [CONCLUSION] DLSS patients with receiving acupuncture, older age, higher SSSQ symptom severity score, and shorter duration of current episode may be more lik
ely to be responder. Duration of current episode modifies the response of acupuncture. These findings provide preliminary evidence to inform patient selection and warrant validation in future studies.

[BACKGROUND] Traumatic brain injury (TBI) is associated with high morbidity, disability, and long-term neurological sequelae. Mitochondrial dysfunction is a central component of secondary injury after TBI, contributing to impaired energy metabolism, oxidative stress, calcium dysregulation, neuroinflammation, and neuronal apoptosis. [MAIN BODY] This review summarizes current evidence regarding acupuncture-mediated regulation of mitochondrial homeostasis after TBI, focusing on structural homeostasis, quantitative homeostasis, and functional/metabolic homeostasis. We distinguish direct evidence from TBI models, indirect evidence from related brain injury models, and hypothesis-generating mechanisms. Particular attention is given to mitochondrial dynamics, mitochondrial biogenesis, mitophagy, energy metabolism, oxidative stress, calcium signaling, mitochondrial membrane potential, apoptosis, intercellular mitochondrial transfer, and putative upstream neural and humoral pathways. [CONCLUSION] Current preclinical evidence suggests that acupuncture, particularly electroacupuncture, may influence mitochondrial homeostasis after TBI through multidimensional and context-dependent mechanisms. However, direct TBI-specific evidence remains limited, and several proposed mechanisms require further validation before acupuncture-mediated mitochondrial regulation can be considered a clinically established therapeutic strategy.

Bell's palsy is the most common cause of acute unilateral lower motor neuron-type facial nerve paralysis and is generally managed with early administration of corticosteroids, with or without antiviral therapy. Although most patients experience substantial recovery, a clinically meaningful subset-including older adults, individuals with delayed presentation, and those with comorbid medical conditions-may be left with incomplete resolution, with persistent facial weakness, asymmetry, or synkinesis. Electroacupuncture (EA) has been proposed as an adjunctive treatment modality for facial nerve recovery, with hypothesized mechanisms including enhanced neuromuscular activation, improved regional blood flow, modulation of local inflammatory responses, and facilitation of peripheral nerve regeneration. However, robust clinical evidence and detailed case-based documentation remain limited. We report the case of an 81-year-old male patient who presented with acute left-sided lower motor neuron-type facial paralysis. The diagnosis was consistent with Bell's palsy, after a cerebrovascular accident was excluded, and neuroimaging demonstrated no acute intracranial pathology. An integrative treatment approach incorporating acupuncture with low-frequency electrical stimulation was initiated on day 17 post-onset, targeting facial and cranial acupoints. EA treatment was administered twice weekly for seven weeks and was supplemented with conservative home-based interventions. Facial nerve function improved from House-Brackmann Grade IV at presentation to Grade I-II. While this case suggests that EA may have a role as an adjunctive therapy in Bell's palsy, including in patients with delayed presentation and less favorable prognostic features, its interpretation is limited by the single-ca
se design and the known potential for spontaneous recovery. Accordingly, controlled prospective studies are warranted to further evaluate the efficacy, optimal timing, and clinical utility of EA in the management of acute facial nerve paralysis.

[OBJECTIVE] To explore the effectiveness and safety of Chinese medicine (CM) treatment based on pattern identification (PI) in elderly patients (⩾ 60 years) with severe community-acquired pneumonia (CAP). [METHODS] This post hoc analysis was based on a multicenter, randomized, placebo-controlled trial. Elderly patients with severe CAP were randomly assigned to receive either conventional medicine plus CM placebo (conventional group) or conventional medicine combined with CM treatment based on PI (combination group) for a 28-d treatment period. The primary outcome was the rate of treatment failure. Secondary outcomes included 28- and 90-d mortality, time to clinical stability, rate of clinical stability, length of hospital stay, and adverse events. [RESULTS] A total of 128 elderly patients with severe CAP were included in the intention-to-treat population, comprising 67 patients in the combination group and 61 in the conventional group. The rate of treatment failure was lower in the combination group than in the conventional group (20.9% vs. 39.3%), with a between-group difference of 18% [95% confidence interval (CI), 0.03-0.34; P<0.05]. The 28-d mortality in the combination group was lower than that in the conventional group (19.4% vs. 36.1%), with a difference of 17% (95% CI, 0.01-0.32; P<0.05). Time to clinical stability was significantly shorter in the combination group than in the conventional group, with a Hazard ratio of 2.44 (95% CI, 1.44-4.14; P<0.01). The rate of clinical stability was higher in the combination group than in the conventional group (64.2% vs. 34.4%), with the between-group difference was 30% (95% CI, 0.13-0.46; P<0.01). 【目的】探讨基于辨证论治的中医药(CM)治疗对≥60 岁老年重症社区获得性肺炎(CAP)患者的有效性与安全性。【方法】本事后分析基于一项多中心、随机、安慰剂对照试验。老年重症 CAP 患者随机接受常规治疗+CM 安慰剂(常规组)或常规治疗+辨证 CM 治疗(联合组),治疗 28 天。主要结局为治疗失败率;次要结局包括 28 天与 90 天死亡率、临床稳定时间、临床稳定率、住院时间与不良事件。【结果】意向性治疗人群共纳入 128 例老年重症 CAP 患者,其中联合组 67 例、常规组 61 例。联合组治疗失败率低于常规组(20.9% vs 39.3%),组间差异 18%(95%CI 0.03-0.34,P<0.05);联合组 28 天死亡率更低(19.4% vs 36.1%),差异 17%(95%CI 0.01-0.32,P<0.05);临床稳定时间显著缩短,HR=2.44(95%CI 1.44-4.14,P<0.01);临床稳定率更高(64.2% vs 34.4%),组间差异 30%(95%CI 0.13-0.46,P<0.01)。符合方案人群中结果相似。两组不良事件发生率相近。【结论】与常规治疗单用相比,常规治疗联合辨证 CM 治疗能有效且安全地降低老年重症 CAP 患者的治疗失败。

[BACKGROUND] Post-stroke fatigue (PSF) is a common and distressing condition among stroke survivors. Chinese herbal medicine (CHM) is frequently utilized in China to manage PSF. However, robust synthesized evidence supporting its efficacy remains limited. This study integrates meta-analysis with medication pattern analysis to establish a comprehensive evidence base for the rational clinical application of CHM in PSF treatment. [METHODS] Relevant RCTs were retrieved from eight databases-including PubMed, Cochrane Library, EMBASE, Web of Science, China National Knowledge Infrastructure Chinese Biomedical Database (SinoMed), Chinese Scientific Journal Database (VIP), and Wanfang-up to March 2026. Meta-analysis was performed using Review Manager 5.4, while funnel plot and Egger's test were utilized to evaluate potential publication bias. Furthermore, we analyzed medication pattern in terms of usage frequency, properties, flavors, meridian tropism, and hierarchical cluster of Traditional Chinese Medicine actions of Chinese medicinal materials. [RESULTS] Nineteen trials involving 1,474 patients were included. Meta-analysis indicated that CHM significantly alleviated fatigue severity, as measured by fatigue severity scale. Evidence also suggested that CHM improved quality of life, motor function, and mood disorders. No significant publication bias was detected. Medication pattern analysis of 15 unique CHM formulas identified 52 Chinese medicinal materials, with high-frequency ingredients including Astragali radix, Angelicae sinensis radix, Chuanxiong rhizoma. The medicinal profiles were characterized by warm or slightly warm natures, sweet and pungent flavors, and tropism toward the spleen, liver, and heart meridians, reflecting core efficacies of tonifying deficiency and acti
vating blood to resolve stasis. [CONCLUSION] This study provides preliminary evidence for the clinical consideration of CHM in the management of post-stroke fatigue and highlights key Chinese medicinal materials and core TCM therapeutic strategies. However, further large-scale, multicenter randomized controlled trials are warranted to strengthen the evidence base and facilitate the integration of CHM into global clinical practice for post-stroke fatigue. [BACKGROUND] Post-stroke thalamic pain (PSTP) represents one of the most challenging types of refractory central neuropathic pain. The application of herbal medicine alongside Western medicine (WM) has been proposed as a potential therapeutic option for managing PSTP. Nevertheless, its comprehensive effectiveness and safety have yet to be conclusively established. [OBJECTIVE] This study aimed to systematically evaluate the efficacy and safety of herbal medicine combined with WM for the management of PSTP. [METHODS] Eight electronic databases were searched from inception to August 2025 to retrieve randomized controlled trials (RCTs) evaluating combined herbal medicine and WM against WM monotherapy in the management of PSTP. Outcomes included pain intensity measures (VAS, PPI, PRI), anxiety and depression scales (HAMA, SAS, HAMD and SDS), overall treatment effectiveness and adverse events. Sensitivity and subgroup analyses were performed to explore potential sources of heterogeneity. The GRADE framework was used to appraise the strength of evidence for specific results. Trial sequential analysis was carried out with TSA software. [RESULTS] The systematic review encompassed 16 trials, incorporating 1,122 participants. The pooled analysis showed that herbal medicine combined with WM was associated with a higher total effective rate than WM alone (RR = 1.38, 95% CI: 1.28-1.49, p < 0.00001). Compared with WM alone, herbal medicine combined with WM significantly reduced VAS score (MD = -1.50, 95% CI: -1.84 - -1.16, p < 0.00001), PPI score (MD = -1.39, 95% CI: -1.55 - -1.22, p < 0.00001), and PRI score (MD = -2.46, 95% CI: -3.24 - -1.69, p < 0.00001). Combination therapy also effectively reduced anxiety (HAMA: MD = -3.10, 95% CI: -3.53 - -2.67, p < 0.00001; SAS: MD = -6.97, 95%
CI: -7.49 - -6.46, p < 0.00001) and depression (HAMD: MD = -2.26, 95% CI: -2.72 - -1.80, p < 0.00001; SDS: MD = -7.15, 95% CI: -7.66 - -6.65, p < 0.00001). Regarding safety, herbal medicine combined with WM was not associated with an increased incidence of adverse events compared with WM alone (RR = 0.95, 95% CI: 0.55-1.64, p = 0.86). The certainty of evidence assessed using GRADE ranged from moderate to very low. TSA provided additional statistical support for the potential benefit of the combined therapy. [CONCLUSION] Evidence from this systematic review indicates that adjunctive herbal medicine combined with WM may have potential benefits in reducing pain, anxiety, and depression, and improving clinical response in patients with PSTP. However, these findings should be interpreted with caution due to methodological limitations and the exclusive inclusion of studies from China. Further rigorous, multicenter, and multinational trials are needed to confirm these findings. [BACKGROUND] By 2050, the proportion of people aged 65 and older is projected to reach 16% globally, up from roughly 10% in 2022. Frailty and sarcopenia are among the most clinically consequential age-related conditions, independently associated with falls, hospitalization, and death. Traditional Chinese medicine (TCM)-encompassing herbal formulas, acupuncture, tai chi, and qigong-has shown therapeutic potential for both conditions, yet no systematic bibliometric overview of this research area exists. [METHODS] Publications on TCM interventions for frailty and sarcopenia were retrieved from the Web of Science Core Collection on 14 March 2026. After a four-phase screening (identification, document-type screening, title/abstract relevance screening, and inclusion), 405 publications were retained. Bibliometric and visualization analyses were performed using CiteSpace (6.2.R6), VOSviewer (1.6.20), and the R package bibliometrix (4.3.0). To avoid tautological bias, all Boolean search terms were removed from the keyword field before every keyword-based analysis (co-occurrence, growth, burst detection, thematic mapping, and thematic evolution). [RESULTS] A total of 405 publications from 2011 to 2026 were included. Annual output grew sharply after 2019 and peaked at 77 publications in 2025. China was the overwhelmingly dominant contributor (910 author appearances), followed by the USA (152) and Japan (131). With search terms excluded, keyword co-occurrence resolved into interpretable clusters: exercise/physical activity/aging; randomized controlled trials of muscle strength and physical function; falls, balance and rehabilitation; evidence synthesis (systematic review/meta-analysis); and cachexia/nutrition/skeletal-muscle inflammation. Burst detection showed early activity in f
alls, balance, elderly, and exercise (2011-2018) shifting toward network pharmacology, molecular docking, ninjin'yoeito, and nutrition (2020-2026). Co-citation analysis identified the seminal frailty-phenotype study as the foundational reference, with a strong tai-chi fall-prevention trial base and the EWGSOP/AWGS sarcopenia consensus definitions as intellectual pillars. [CONCLUSION] TCM research on frailty and sarcopenia has grown substantially, particularly since 2019, with China as the primary driver and Japan and the USA as secondary contributors. Network pharmacology, nutrition-based intervention science, and oxidative-stress/inflammation mechanisms represent the most promising emerging frontiers. These findings provide a structured evidence map for prioritizing future research and translational efforts in TCM-based geriatric care.

Gut microbiota exert core regulatory effects on systemic metabolic homeostasis and immune balance under physiological and pathological conditions. Marked gut dysbiosis occurs during the progression of chronic kidney disease (CKD), characterized by abnormal enrichment of opportunistic pathogens and depletion of beneficial commensal bacteria. This pathological alteration leads to excessive accumulation of uremic toxins including indoxyl sulfate, p-cresyl sulfate and trimethylamine N-oxide (TMAO), as well as sharp reductions in the synthesis of intestinal protective metabolites such as short-chain fatty acids (SCFAs) and indole-3-aldehyde. The imbalance between harmful and protective metabolites persistently triggers renal inflammatory activation and interstitial fibrosis via activating multiple signaling pathways, including the renin-angiotensin system, oxidative stress, Toll-like receptor 4, aryl hydrocarbon receptor, IκB/NF-κB, Keap1/Nrf2, NLRP3 inflammasome and ferroptosis. This review systematically summarizes the pathological correlations between gut microbiota disturbance and various renal disorders, namely diabetic kidney disease, membranous nephropathy, lupus nephritis, IgA nephropathy, hypertension-associated nephropathy and dialysis-related renal injury, and clarifies the universal mechanisms and disease-specific characteristics underlying gut-kidney axis-mediated progression of different subtypes of kidney diseases. Natural products represented by natural polysaccharides, traditional Chinese medicine formulas and plant polyphenols are capable of remodeling gut microbiota composition, repairing intestinal mucosal barrier function, balancing microbial metabolic profiles and regulating gut-kidney axis homeostasis, thereby exerting renal and multi-organ protective
effects in patients with CKD and dialysis-dependent individuals. Targeted modulation of intestinal microecology and intervention against gut-kidney axis dysfunction using natural products provide novel strategies for early prevention and control of CKD, disease progression retardation and prognosis improvement in dialysis patients, and also lay a theoretical foundation for the development and clinical translational application of novel therapeutic targets for kidney diseases.

[PURPOSE] Ischemic stroke poses a major global health burden, and growing evidence points to the microbiota-gut-brain axis as an important contributor to stroke pathophysiology and recovery. The purpose of this review is to summarize current knowledge on bidirectional gut-brain communication in stroke, establishing gut dysbiosis as an upstream risk factor for stroke onset and examining microbiota dynamics across acute and chronic phases. [METHOD] A comprehensive review of the current literature was conducted to synthesize existing evidence on the mechanisms of gut-brain communication, the influence of host factors, post-stroke complications, and candidate therapeutic interventions targeting the MGBA in ischemic stroke. [FINDING] Gut dysbiosis acts as a risk factor for stroke through TMAO-mediated atherothrombosis, systemic inflammatory priming, and comorbidity-associated dysbiotic pre-conditioning. Following a stroke, dysbiosis is characterized by a depletion of short-chain fatty acid (SCFA)-producing taxa and an expansion of pro-inflammatory bacteria, particularly Proteobacteria and Enterobacteriaceae. Host factors (age, sex, genetics, and comorbidities) shape microbiota signatures and influence outcomes. Furthermore, post-stroke complications-including cognitive impairment, depression, stroke-associated pneumonia, and gastrointestinal dysfunction-are intimately linked to persistent dysbiosis. Mechanistically, the gut microbiota modulates stroke outcomes through immune regulation, metabolite signaling, blood-brain barrier integrity, and vagal pathways. Candidate therapeutic interventions (probiotics/prebiotics, fecal microbiota transplantation, SCFA supplementation, traditional Chinese medicine, and neuromodulation) demonstrate promising neuroprotective and recovery-en
hancing effects, predominantly in preclinical models. [CONCLUSION] While targeted interventions like probiotics and dietary fiber supplementation have shown modest benefits in small clinical trials, human clinical evidence remains largely preliminary. The translation of FMT, SCFA supplementation, and neuromodulatory approaches to clinical practice requires validation through rigorous, adequately powered randomized controlled trials. Future precision microbiome medicine demands individualized profiling and phenotype-specific interventions to establish microbiota-targeted strategies as viable adjunctive stroke therapies.

英文原摘要(English Abstract)
[BACKGROUND] Acupuncture is a complementary medicine therapy increasingly used to manage cancer-related symptoms. Despite its demonstrated efficacy, adherence rates among patients with breast cancer remain suboptimal. This study aimed to identify factors associated with non-completion of acupuncture regimens within a single institutional integrative oncology program to identify modifiable barriers to service utilization. [METHODS] Patients with breast cancer who began acupuncture treatment at an NCI-designated comprehensive cancer center between May 2022 and March 2025 were retrospectively included in this study. Regimen completion was defined as attending a minimum of eight sessions. Demographic, clinical, and patient-reported outcomes (PROMIS-29) were analyzed. Stepwise multivariable logistic regression was used to identify predictors of regimen completion. [RESULTS] Of 563 patients, 268 (47.6%) completed the recommended acupuncture regimen. In multivariable analysis, non-White race (OR 0.64, 95% CI 0.45-0.92, p = 0.016), advanced cancer stage (defined as stages 3-4; OR 0.55, 95% CI 0.37-0.82, p = 0.003), smoking history (OR 0.63, 95% CI 0.41-0.95, p = 0.029), and moderate-to-severe anxiety (OR 0.67, 95% CI 0.45-1.00, p = 0.048) were independently associated with lower odds of regimen completion. [CONCLUSION] Racial disparities, clinical stage, baseline anxiety, and smoking history were significantly associated with lowered acupuncture adherence in patients with breast cancer. Tailored interventions including early identification of at-risk patients and integrated mental health support may be evaluated to improve adherence to integrative therapies.
